** If you have previously participated in a Sport at East or Horace Mann please click View My Account in the upper right corner using the Username and Password you created the first time you registered.**

**This will automatically fill in the Registration Form so you will only need to update Coaches Expectations.

If you have already registered 1 child through this system, please add additional family members via your existing account (returning users).

ALL PAPERWORK MUST BE FULLY COMPLETED AND ELECTRONICALLY SUBMITTED PRIOR TO PARTICIPATION AT ANY LEVEL, IN ANY PRACTICES - NO EXCEPTIONS

1. Each student athlete must have a completed WIAA Sports Physical Card on file at WEHS. Note: Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. 2023-2024 sports physicals must be dated 4-1-2022 or later. 
2. WIAA Alternate Year Cards are to be completed every other year. 
3. Parents & Athletes must read the  Wausau Schools Code of Conduct then submit the completed Athletic Code Signature Form & Parental Insurance Waiver once per school year. 
4. An Agreement of Coaches Expectations e-signed and dated by parent/guardian & student for each sport that you participate in is required.
5. WIAA required Concussion Form needs to be completed once per school year.
6. An Emergency Information Card must be submitted to have on file for medical needs. This card must be completed prior to participation in the first sports season of the year for each student athlete. 
7. Activity Fee Payment ($50 per sport, $75 for Lacrosse is required for each sports season. Envelopes for payment are available in the business office or athletic office. Payments can be turned in to either office.

Note: If you are participating in multiple sports, an activity fee for each sport will need to be paid. An Agreement of Coaches Expectations will also need to be turned in for each sport.
